HMC Health & Rehabilitation Center is a dual-certified 73 bed facility, offering both skilled care and traditional nursing home care. The dual certification allows HMC Health & Rehabilitation Center to accept residents that are in need of specialized skilled nursing care as well as traditional nursing home residents. A skilled nursing facility (SNF) is a place of residence for people who require constant nursing care and have significant deficiencies with activities of daily living. Residents include the elderly and younger adults with physical disabilities.
We strive to make our facility an environment of caring individuals focused on support and care of the residents in our care. HMC Health & Rehabilitation Center is a Joint Commission accredited facility.

At HMC Health & Rehabilitation Center residents have the right to the best medical care available. We have onsite rehabilitation services including speech, occupational and physical therapies, easy access to 24-hour emergency care with physician coverage, psychiatric services, and a pharmacy. Our onsite registered dietician can assist with any special dietary needs that our residents have. We also offer easy access to Outpatient Clinics at Hardin Medical Center including Tele-Medicine consultations which have dermatology and other specialties available.
Home and community based services are one of the ways that HMC Health & Rehabilitation Center is expanding its offering to increase the choices that families have. Homes and community based services include:
Adult Day Care
A place where you can go for more than 3 hours but less
than 24 hours a day to spend
time with others.
Assistive Technology
Devices to help you with
activities of daily living, such
as grabbers or big-handled
eating utensils.
Home-Delivered Meals
One healthy meal each day
delivered to your home.
Homemaker Services
Someone to help you with
household chores or errands,
like laundry, sweeping or
grocery shopping.
In-Home Respite
Someone to come and stay
with you in your home for a
short time so your caregiver
can get some rest.
Inpatient Respite
Short stay in a nursing home
so your caregiver can get
some rest.
Personal Care Assistant
Someone to help you with
activities of daily living for
longer periods of time or go
with you to doctor visits or
other appointments.
Personal Care Service
Someone to help you with
activities of daily living, such
as bathing, toileting or
transfers.

Our residents have the right to privacy, respect, and confidentiality. They also have the right to have their pain managed and we strive to make each and every resident's life comfortable.
We offer a variety of valuable resources for our resident's emotional and spiritual needs. We have on-call pastors available. Our activities director schedules various activities and entertainment for our resident's enjoyment. Click HERE for the upcoming month's activities. We encourage all of our residents who has specific cultural or spiritual beliefs to talk with our social worker so we can accommodate their needs. There is also a chapel located on the annex level of the hospital available for resident use.

At HMC Health & Rehabilitation Center we encourage our residents and their families to be involved in their care and planning. Every three months we encourage family to join us in reviewing the resident's plan of care, and always encourage that all questions or concerns be brought to our attention.
