Important Employee Health Coverage Tax Documents

The 1095 tax form(s) for 2024 are prepared and available upon request. Form 1095 provides information about coverage offers made to full-time employees and details about coverage for those enrolled in the group health plan at Hardin Medical Center, located at 935 Wayne Road in Savannah, TN.
